REGISTRATION FORM  FOR PHIL PERRY VALENTINES' WEEKEND CRUISE 2009
This contract includes very important information to the passenger (“Participant”) from Jazz Fest Travel (“Operator”). Payment of a deposit by the participant for a package shall represent consent and acknowledgment of all provisions in the condition list as published in this contract. All checks and money orders must be made payable to “Jazz Fest Travel, Inc."
Reservations/Deposits: A deposit in the amount of $100.00 per person, a signed contract, and a reservation form are due to Jazz Fest Travelwithin 5 days of making your reservation. Upon receipt of the above items your reservation becomes confirmed and will be under penalty for any cancellations or changes. Please forward all payments to: Jazz Fest Travel, Inc., P.O. Box 10126, Gaithersburg, MD 20898.
Final/Full Payment: Final payment is due 60 days prior to departure. Reservations made less than 60 days prior to departure will require full payment, a signed contract, and a reservation form at time of booking. Bookings made that do not meet the Deposits and Final Payment requirements listed in this contract will be canceled without refund.
Insurance: We strongly recommend insurance to safeguard yourself against cancellation charges and additional expenses you may incur before the departure of your trip. Insurance provides protection for non-pre-existing conditions. Once purchased this feature is non-refundable. Trip Cancellation /Interruption Insurance covers the total cost of the land tour for accidental injury, sickness, death, jury duty, fire or flood to home for the passenger insured and/or their immediate family members.
Cancellations & Changes: Penalties apply to the cancellation of both land products and transportation products. There is a $100.00 per person fee for any cancellations, once the deposit is paid. Transportation penalties are calculated separately. Tickets for any shows purchased are non-refundable. If an entire booking is cancelled between April 1, 2008 and Aug. 18, 2008, the minimum penalty will be $100.00. If a land product is cancelled within 60 days of departure, a cancellation penalty applies. Name changes are subject to hotel and airline acceptance and penalties.
The amount of the penalty depends on how close to departure that you cancel as per the chart below:
Cancellation Penalties:
60-30 Days prior to departure: $300.00 per person
29 - 15 Day prior to departure: 50% of payment received or less:
14 days or less: No Refund
Documentation: Passengers traveling outside the United States are required to carry proof of citizenship. U.S. citizens must have a government issued current passport, or an original or certified copy of their Birth Certificate with a raised seal plus government issued photo identification. Hospital issued Birth Certificates are not valid.
Responsibility: The Operator and/or their agents liability is limited. They act as agents for the Participant in relation to travel and accommodations whether by motorcoach, private car, boat, aircraft or any other mode of transportation. The Operator, and its parent, subsidiaries, and affiliates will not be responsible or liable for injury, personal injury, property damage, property and other losses, accident, delay or irregularity which may be caused either by reason or defect, through the acts or default of any company or person contracted in conveying passengers or in carrying out the arrangements of the tour, or as direct or indirect acts of God, dangers of the sea, the Year 2000 Problem, fire, breakdown of machinery or equipment, acts of governments of other authorities, or from any other cause beyond the Operator’s control. Artists are subject to change and therefore Jazz Fest Travel and IAI is not to be held responsible for any such changes. The Operator reserves the right to decline to accept any participant as a member of the tour, or to cancel a tour if necessary. The sole and exclusive responsibility and liability of any airline used in the Operator’s program shall be in accordance with and as limited by the contract of a carriage in the participant’s ticket, the applicable tariffs, and the provisions of the Warsaw Convention.
